Job summary

Equiliem Healthcare is seeking a CANS Assessor to coordinate and complete comprehensive assessments using a person-centered approach across Greene, Ulster, and Delaware counties. The role blends in-person field work with telehealth, requiring strong analytical and communication skills and the ability to work with caregivers, care managers, and providers.

Qualifications

  • Minimum of one year working with individuals with intellectual and/or developmental disabilities.
  • Willingness to travel throughout the assigned region with a reliable vehicle and insurance.
  • Some evening and weekend hours required.

Responsibilities

  • Outreach to individuals and caregiver/primary supports as necessary.
  • Schedule and coordinate assessment process with individuals, caregiver/primary support, and Care Managers/providers in home/community or via telehealth.
  • Schedule and conduct assessments at locations/times convenient for individuals.
  • Use an in-person, person-centered interview approach.
  • Interview caregivers/primary supports; verify information with records before finalizing assessments.
  • Arrange interpreter services as needed.
  • Complete assessments using provided software and code assessments per training.
  • Document all case activity per program requirements.
  • Meet or exceed monthly productivity standards.
  • Ensure assessments meet quality standards and guidelines.
  • Perform other duties as assigned by Management.

HYBRID ROLE: IN-PERSON AND VIRTUAL VISITS - Counties: Greene, Ulster, and Delaware County
Job Summary

The CANS Assessor is responsible for arranging, conducting, and completing accurate and thorough assessments using a person-centered approach.

Job Responsibilities
  • Outreach to individuals and their caregiver/primary supports as necessary during assigned schedule
  • Schedule and coordinate the assessment process with individuals, caregiver/primary support, and Care Managers/providers in the home and/or community or via telehealth using a person-centered approach
  • Schedule and conduct the assessment at a location and time which is most convenient for the individual
  • Utilize an in-person, person-centered interview process as part of the assessment process (e.g., conversational style that starts with the person)
  • Conduct additional interviews with caregiver/primary support and utilize multiple sources including records to verify information and coding prior to finalizing the assessment
  • Arrange formal interpreter services as needed
  • Complete all assessments utilizing the provided software
  • Code the assessment accurately based on training
  • Document all case activity as per program requirements
  • Meet or exceed established monthly productivity standards
  • All assessments must meet quality standards (e.g., assessment fully completed, time frames of assessment maintained, etc.)
  • Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position
  • Perform other duties as may be assigned by Management.

Experience

Minimum of one year working with individuals with intellectual and/or developmental disabilities in a full-time position; experience in adapting interviewing skills to elicit information from individuals with intellectual and/or developmental disabilities, their families, caregivers and staff, and various clinicians. Experience working with children with intellectual and/or developmental disabilities and their families and knowledge of the supports and services available. Experience: Pediatric I/DD: 1 year (Required)

Required Education

Bachelor's degree in an approved human service discipline

Travel

Must be willing and able to travel throughout the assigned region. Assessors outside of the NYC 5 boroughs must have a reliable vehicle with a valid driver's license and insurance.

Scheduling

Must be willing and able to accommodate varying schedules and hours; some evening and weekend hours required.

Expected hours: 40 per week

Schedule
  • 8-hour shift
  • Monday to Friday
  • Weekends as needed.
