Illumination Health + Home in Anaheim, CA is seeking a Medical Assistant/EMT to support discharge planning and ongoing home health care. The role requires a CA MA certificate or EMT license, 2 years of relevant experience, and bilingual English/Spanish.

You will perform intakes, assess patient status, coordinate visits, educate clients, document in KIPU, and maintain medication logs while ensuring HIPAA and OSHA compliance. This is an on-site position.

Formación

• High School diploma or equivalent required.
• Current MA certificate or EMT license required.
• 2 years of relevant healthcare experience.
• Bilingual English/Spanish required.
• Must be able to use email and basic spreadsheets.

Responsabilidades

• Perform initial intakes to ensure proper client discharge and document findings.
• Assess medical and mental health status upon arrival, including vitals and insurance status.
• Coordinate home health visits and follow-up MD appointments.
• Link clients to medical services and arrange transportation when needed.
• Provide patient education to promote health literacy under supervision.
• Enter client data into KIPU and other systems; maintain medical logs.
• Maintain med room and supply inventory; ensure OSHA compliance.
