Conviva-Primary Care Physician- Tampa, FL

Join our team as a Primary Care Physician at Conviva Senior Primary Care in Tampa, FL. This full-time role offers a competitive salary and the opportunity to work in a value-based care environment focused on quality and patient-centered services.

Position Details

• Base pay range: $203,400.00/yr - $299,500.00/yr
• Scheduled weekly hours: 40

Responsibilities

• Evaluate and treat patients according to standards of care.
• Coordinate patient services, including referrals, hospital and SNF coordination, and home health care.
• Participate actively in team-based care and daily huddles.
• Complete timely medical record documentation and adhere to policies.
• Engage in quality improvement and potential service growth initiatives.
• Provide direct patient care, including face-to-face consultations and administrative tasks related to patient care.

Qualifications

Required

• Current and unrestricted medical license or willingness to obtain in Florida.
• Graduate of accredited MD or DO program.
• Excellent communication skills and interpersonal relationships.
• Experience with integrated, team-based care models.

Preferred

• Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active DEA license and Medicaid Provider Number.
• Experience managing Medicare Advantage panels and knowledge of Medicare guidelines.
