Job summary

Kids for the Future is seeking a Contracted Occupational Therapist to provide in-home skilled OT services under a 1099 agreement. You will evaluate patients, develop treatment plans, implement interventions, and collaborate with an interdisciplinary team to maximize independence and safety.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in the state of practice.
• Minimum of one year of clinical experience; home health experience preferred.
• Current CPR certification.
• Valid driver's license, reliable transportation, and proof of automobile insurance.
• Strong clinical assessment, communication, documentation, and organizational skills.
• Ability to work independently while collaborating with the interdisciplinary care team.

Responsibilities

• Perform comprehensive occupational therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans based on physician orders and patient needs.
• Provide skilled OT interventions to improve ADLs, IADLs, cognition, and mobility.
• Evaluate home environment and recommend modifications or adaptive equipment.
• Educate patients, families, and caregivers on home exercise programs and safety.
• Reassess progress and modify treatment plans as appropriate.
• Coordinate care with physicians, nurses, PTs, SLPs, social workers, and home health aides.
• Complete accurate documentation of evaluations, visits, progress notes, and discharge summaries.
• Participate in care conferences and communicate changes promptly.
• Maintain compliance with HIPAA, OSHA, Medicare, and state regulations.

Job Summary

The Contracted Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes under a contractual agreement with the home health agency. The OT evaluates patients, develops individualized treatment plans, implements therapeutic interventions, and collaborates with the interdisciplinary team to maximize patients' independence, safety, and quality of life. Services are provided in accordance with physician orders, agency policies, state practice acts, and applicable Medicare and regulatory requirements.

• Perform comprehensive occupational therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans based on physician orders and patient needs.
• Provide skilled occupational therapy interventions to improve activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, cognition, coordination, strength, balance, and functional mobility.
• Evaluate the home environment and recommend modifications or adaptive equipment to enhance patient safety and independence.
• Educate patients, family members, and caregivers on home exercise programs, adaptive techniques, fall prevention, and energy conservation.
• Reassess patient progress and modify treatment plans as clinically appropriate.
• Coordinate care with physicians, nurses, physical therapists, speech-language pathologists, social workers, home health aides, and other members of the interdisciplinary team.
• Complete accurate, timely, and compliant documentation of evaluations, treatment visits, progress notes, recertifications, and discharge summaries.
• Participate in care conferences and communicate changes in patient condition promptly.
• Maintain compliance with agency policies, HIPAA, OSHA standards, Medicare Conditions of Participation, state regulations, and professional practice standards.
• Maintain current licensure, certifications, and required continuing education.

Contracted Position Requirements

• Functions as an independent contractor and is responsible for managing assigned patient visits within agreed-upon timeframes.
• Maintains professional liability (malpractice) insurance.
• Provides reliable transportation and travels to patient homes throughout the agency's service area.
• Supplies documentation within agency-established deadlines.
• Maintains all required credentials and background clearances as required by federal, state, and agency policies.

Compensation

This is a contract (1099) position. Compensation is provided on a per-visit or negotiated contractual basis in accordance with the terms of the independent contractor agreement.
