Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier care in home health across a defined CA area. You will assess and address ADLs, functional status, and safety, then create and implement plans that enhance wellbeing.

As an Independent Contractor, you’ll enjoy scheduling autonomy, above-average compensation, and strong administrative support to coordinate care and ensure quality outcomes.

Qualifications

• Ability to work with adults of all ages in home health settings.
• Occupational Therapist license and state registration.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Assess patient care needs related to functionality, ADLs, fine motor skills, and home safety.
• Develop treatment plans and implement interventions to improve wellbeing.
• Evaluate patient progress and adjust plans as needed.
• Lead therapeutic activities and teach self-care tasks to patients.
• Observe, record, and report responses to treatment and changes in condition.
• Instruct patients, families, caregivers, and health team on therapy participation.

This position is for an Independent Contractor to serve a specific area.

FeldCare Connects is currently seeking a self‑motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient’s wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self‑care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapist license and registration by the state
• Completion of an accredited Occupational Therapist program
