Job summary

SnapCare is seeking a compassionate Speech Language Pathologist (SLP) for a 13-week contract in Bishop, CA. The role involves assessing, diagnosing, and treating speech, language, cognitive-communication, voice, and swallowing disorders within an interdisciplinary healthcare team.

Responsibilities include developing individualized treatment plans, conducting bedside swallow evaluations, educating patients and caregivers, and maintaining detailed documentation.

Qualifications

• Active state licensure as a Speech Language Pathologist.
• CCC-SLP certification is preferred or required based on facility requirements.
• Current CPR/BLS certification preferred.
• Experience in acute care, outpatient, rehabilitation, skilled nursing, school, or home health settings preferred.

Responsibilities

• Evaluate patients to assess speech, language, cognitive-communication, voice, and swallowing disorders.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Provide therapy to improve communication skills, speech clarity, language development, cognitive functioning, and swallowing abilities.
• Conduct bedside swallow evaluations and recommend dietary modifications when appropriate.
• Educate patients, families, and caregivers on treatment plans, home exercises, and communication strategies.
• Monitor patient progress and adjust treatment interventions as necessary.
• Maintain accurate and timely patient documentation, including evaluations, progress notes, treatment plans, and discharge summaries.
• Collaborate with physicians, nurses, occupational therapists, physical therapists, and other healthcare professionals to coordinate care.
• Ensure compliance with facility policies, regulatory standards, and patient safety guidelines.
• Participate in care planning and interdisciplinary team meetings as needed.
