Job summary

Centra Healthcare Solutions is seeking a licensed Physical Therapist (PT) in Farmington, MN for a contract assignment. The role involves delivering skilled PT services, developing plans of care, and collaborating with the care team to achieve patient goals.

Must be a graduate of an accredited program with an active PT license and CPR certification. SNF/LTC setting experience is a plus.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT).
• Current CPR certification.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of PT interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
