Job summary

Centra Healthcare Allied seeks a travel Physical Therapist for a contract assignment in Farmington, Minnesota. The role is a SNF/LTC setting with 13-week duration, 40 hours per week, and an 8-hour shift.

Ideal candidates have graduated from an accredited program, hold (or be in the process of) state PT licensure, CPR certification, and can develop/execute plans of care per physician orders. Travel could involve relocation within the United States.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals.
