Job summary

The selected candidate will join a SNF/LTC setting and provide skilled PT services per physician orders, develop plans of care, assess patient needs, and collaborate with interdisciplinary teams.

Requirements include graduation from an accredited program, current WI PT license (or in process) and CPR certification.

Qualifications

• Must be licensed as a Physical Therapist (PT).
• Graduated from an accredited school.
• CPR certification current.
• Develop plan of care for each PT patient.
• Provide skilled PT services per physician orders.
• Assess, plan, evaluate and modify care to meet PT goals.
• Collaborate with all disciplines to achieve patient goals.
