Qualifications

• Minimum 1 year of PT experience and active PT license.
• Ability to work in a Skilled Nursing Facility setting.
• Reliable transportation and punctual attendance.
• Legal authorization to work in the United States.
• Willingness to complete background and drug screening.

Responsibilities

• Document assessments and treatments in EMR and record vital signs.
• Evaluate residents' mobility and rehabilitation needs.
• Develop and implement individualized treatment plans to improve function.
• Provide therapeutic exercises, gait and balance training.
• Educate residents and caregivers on exercises and fall prevention.
• Monitor progress and adjust plans based on goals.
• Maintain compliance with facility policies and standards.
