Job summary

Centra Healthcare Allied seeks a Physical Therapist for a travel assignment in Greeley, Colorado. The role is a 13-week contract, 40 hours per week, days shift, in a SNF/LTC setting, with travel required to and within Colorado.

The candidate must be a licensed PT, have current CPR, and be able to develop and implement plans of care while delivering skilled therapy per physician orders and collaborating with the care team to achieve patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT).
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Collaborates with all disciplines to plan and evaluate team goals.

Responsibilities
