The role focuses on delivering high-quality PT services under physician orders and collaborating with a multidisciplinary team to achieve patient goals.

The ideal candidate has graduated from an accredited PT program, holds a current PT license in the state, and maintains CPR certification.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Current state license as a Physical Therapist (PT) or actively pursuing licensure.
• CPR certification.
• Ability to develop a plan of care for each patient.
• Able to provide skilled physical therapy services per physician orders.
• Collaborates with multidisciplinary team to meet patient goals.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of PT interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
