Qualifications

• Active and unencumbered IN PT License or Compact PT License.
• Minimum 1 year of experience in skilled nursing.
• Must have reliable transportation to assigned facilities.
• Must maintain consistent attendance and punctuality.
• Able to push, pull, and lift at least 50 pounds.
• Must be legally authorized to work in the United States.
• Willing to complete background and drug screening.

Responsibilities

• Record patients’ medical information and vital signs in EMR.
• Evaluate residents’ physical function, strength, balance, mobility, and rehab needs.
• Develop and implement individualized treatment plans to improve mobility and independence.
• Provide therapeutic exercises, gait and balance training, and other PT interventions.
• Recommend and train residents in mobility aids and equipment as appropriate.
• Monitor progress and adjust treatment plans based on goals and findings.
• Educate residents, families, and caregivers on exercises and fall prevention.
• Accurately document evaluations, treatments, and outcomes.
• Maintain compliance with facility policies and standards.
• Communicate concerns or issues to onsite supervisor and agency.
• Other duties as assigned.
