Centra Healthcare is seeking an Occupational Therapist (OT) for a SNF/LTC contract in Palestine, TX. The role involves delivering OT services under physician orders and collaborating with a multidisciplinary team to support patient goals.

This contract position offers daily responsibilities and a schedule based on the site needs. Must be a graduate of an accredited OT program with current licensure or in process.

Qualifications

• Graduated from an accredited OT program.
• Current state OT license or in process for licensure.
• Provide skilled occupational therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet OT goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
