Job summary

Centra Healthcare Solutions is seeking an Occupational Therapist (OT) for a contract assignment in a SNF/LTC setting. The OT will assess referred patients, establish and administer treatment programs, and document patient progress under physician direction.

Must have graduated from an accredited OT program, hold or be in process for state licensure, and maintain CPR certification. Collaboration with disciplines and patient/family education are key.

Qualifications

• Graduated from an accredited Occupational Therapy program.
• Current state license or in the process of licensure as an OT.
• Current CPR certification.
• Provide skilled occupational therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet OT goals.
• Collaborate with all disciplines to plan and evaluate patient goals.

Responsibilities
