Job summary

Sweetwater Care in Porterville, CA is seeking a Licensed Vocational Nurse (LVN) supervisor to oversee nursing care within the facility’s policies and physician orders. You will collaborate with physicians and care teams to ensure timelyMed administration and treatments while maintaining patient safety.

Candidates must hold a California LVN license; management experience is preferred, with long-term care and PCC charting familiarity considered a plus. This role is on-site in Porterville, CA.

Qualifications

• Licensed Vocational Nurse (LVN) in California is required.
• Nursing leadership/management experience preferred (not mandatory).
• Experience in LONG TERM CARE setting is required or strongly preferred.
• POINT CLICK CARE (PCC) charting experience is preferred.

Responsibilities

• Provides nursing care within scope of practice according to standards and physician orders.
• Implements and participates in the Quality Improvement Program.
• Uses the Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (rooms, admits, discharges).
• Educates patients and families about care plans.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders as allowed by the Nurse Practice Act.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
