CONTRACT - LICENSED VOCATIONAL NURSE (LVN) PM SHIFT, NOC SHIFT

Position Description

Provides nursing care within scope of practice to ensure patients' needs are met in accordance with standards of practice, physician orders, center policies, and state, federal, and local guidelines.

Qualifications

• Long Term Care experience
• Point Click Care charting experience
• Current LVN/LPN license in practicing state
• Current BLS/CPR certification or willingness to acquire one
• Experience in nursing management is preferred but not required

Responsibilities

• Follow established procedures for charting and incident reporting
• Follow company procedures for narcotic counting, pharmacy orders, and restraint reduction
• Document any change in patient's conditions, including nutritional problems, infections, skin conditions, weight loss, or other noticeable changes
• Utilize the Quality Life Manual for patient identification, placement, and monitoring in appropriate programs
• Document and notify relevant departments of room changes, LOA, new admits, or discharges
• Make rounds with physicians as necessary
