Job summary

Sweetwater Care is seeking an LVN in California to provide nursing care within the scope of practice and physician orders. The role emphasizes infection control, patient education, and active participation in quality improvement initiatives in a SNF/LTC setting.

The incumbent will monitor patients, communicate changes across departments, and assist with medication administration and treatments in accordance with facility policy and state regulations. PCC charting experience is preferred.

Qualifications

• Knowledge and use of infection control standards and policies.
• Participation in the Quality Improvement Program per facility guidelines.
• Education provided to families and patients as needed.
• Experience documenting and communicating through PCC charting.

Responsibilities

• Provides nursing care within scope of practice per standards and orders.
• Notifies departments of patient changes and facilitates transitions.
• Conducts patient and family education on care plans.
• Administers medications and treatments timely per policy.
• Receives, transcribes and carries out physician orders as allowed by law.
• Might make rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
