Description

Position: Clinical Pharmacist - GA or TN (1099 Contractor)

The Clinical Pharmacist implements Monogram’s medication therapy management (MTM) program, ensuring appropriate selection and use of medications by our patients. As a 1099 contractor, the Pharmacist performs timely written comprehensive medication reviews (CMRs) using patient medication lists, prescription claims, and clinical data. They create drug therapy plans, communicate recommendations to providers, and conduct telephonic consultations with medical providers and patients. The Pharmacist collaborates with local clinical staff to optimize patient medication use and outcomes.

Roles And Responsibilities
  • Perform and document comprehensive medication reviews (CMRs) adhering to standards.
  • Drive medication interventions to promote adherence to evidence-based guidelines.
  • Review medical records for drug therapy planning and quality assurance.
  • Serve as a resource for local clinicians regarding medication use.
  • Assist in delivering MTM training to clinical staff.
  • Monitor medication use trends and stay current with prescribing guidelines for kidney disease and related conditions.
  • Participate in interdisciplinary team meetings and share CMR documentation.
  • Implement medication optimization strategies tailored to patient needs.
  • Support organizational tasks to deliver quality medication management services.
  • Uphold Monogram’s mission and values.
Position Requirements
  • Bachelor’s degree in pharmacy or PharmD.
  • Active pharmacist license in the state of residence.
  • At least 3 years of practice experience, preferably in managed care or clinical settings.
Desired Qualifications
  • PGY1 residency in ambulatory or managed care pharmacy.
  • BPS certification in pharmacotherapy, geriatrics, or ambulatory care.
  • Board certification in MTM from NBMTM.
  • MTM training from APhA or similar.
  • Location-based work in Brentwood, TN, or full remote with some travel.
