A leading healthcare provider is looking for a Clinical Pharmacist 1 for a temporary remote position. This role involves daily outreach to address medication adherence gaps, focusing on various HEDIS measures, and includes responsibilities that ensure quality improvement across the board.
Day-to-Day Responsibilities and Project Description (Outside of Workday JD):
YEP/HEDIS - Working Excel reports for member/provider outreach around medication adherence gap closure for multiple Quality HEDIS measures. Some will have a focus around SPC/SPD, and others could have a focus around Behavioral Health measures. CMS STARS adherence measures for Medicare may also be a focus.
Performance Expectations/Metrics:
Performance expectations include completing outreaches daily to members/providers/pharmacy for gap closures (phone and fax). Metrics depend on volume and which HEDIS measure is being worked.
