Job summary

Sage GroupHealthCare Staffing is seeking an LPN for Home Health in Lynn Haven, FL on a 13-week travel/contract assignment. Local and travel candidates are welcome. The role offers 40 hours per week, five shifts, Mon-Fri, with mileage reimbursement and a travel/housing stipend for eligible travelers.

Requirements include 2 years LPN experience, 1 year home health in the last 3 years, OASIS and HCHB proficiency, FL LPN license or compact, and BLS. ASN required; BSN preferred. Start ASAP.

• Must have valid Driver’s License and Personal Auto Insurance

Responsibilities

• Provide and coordinate care under supervision for assigned patients in comfort of their homes
• Support patient recovery and ability to stay at home - enabling them to spend quality time with loved ones
• Empower patients and families to reach healthcare goals by educating them on disease management
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented
• Notify physicians of significant changes in status
• Document and complete charting for all patient visits within 24 hours
• Territories to cover: Panama City and surrounding counties
