Job summary

Sage GroupHealthCare Staffing seeks a Registered Nurse Home Health for a travel/contract assignment in Girard, OH. The role is full-time with 13-week length, accepting local and traveler candidates. Pay range is $1,960–$2,040 weekly gross, with non-taxable travel/housing and M&I stipend for eligible travelers.

Unit is Home Health. Shift is 8 hours, days 8a–5p; 40 hours per week, Mon-Fri. Travel reimbursement and OASIS/HCHB experience preferred.

Qualifications

• Registered nurse with home health experience required.
• OASIS experience required.
• Must have valid driver’s license and auto insurance.
• Covid vaccinated or exemption.
• RN license for OH or Compact.

Responsibilities

• Provide and coordinate care for Home Health patients at home.
• Develop and implement nursing care plans including Start of Care and admissions.
• Support patient recovery and enable home-based living.

• Provideand coordinate care for assigned Home Health patients in comfort of their homes.
• Developand implement nursing care plans including Start of Care and admissions asassigned.
• Supportpatient recovery and ability to stay at home - enabling them to spend qualitytime with their loved ones and doing the activities they enjoy.
• Empowerpatients and families to reach healthcare goals by educating them on diseasemanagement.
• ProvideHome Health nursing care and services within the standards and scope of nursingpractice as defined within the state Nurse Practice Act regulations, policies,procedures, and protocols, as well as facility and corporate policies andprocedures.
• Ensurephysicians’ orders are legible if taken verbally and transcribed appropriatelyper policy and procedure.
• Ensurepatients’ status is assessed on an ongoing basis and pertinent information isgathered and documented.
• Notifyphysicians of significant changes in status.
• Documentand complete charting for all patient visits within 24 hours.
• Providebereavement support to patient’s families as needed
