Position Requirements

• Current Occupational Therapist experience: 1-2 years - Required
• Recent/Current Home Health OT experience: 12 months within the past 3 years - Required
• Home Care Home Base (HCHB)charting experience - Required
• Must have valid Driver’s Licenseand Personal Auto Insurance
• Must be Covid Vaccinated orprovide medical or religious exemption

Education, Licenseand Certification Requirements

• Bachelor of Science degree in Occupational Therapy or Similar – Required

Job Description

• Provide Occupational Therapy services to patients/clients at their homes.
• Consult with the physician and an interdisciplinary team in thedevelopment of a plan of treatment
• Utilize skills in patient/client evaluation and treatment, professionalconsultations to achieve maximum patient/client independence in accordance withthe individual's functional level and potential
• Plans and prepares treatment plan based on evaluation of patient data
• Provide in-home OT treatment programs to assist patients in restoringfunction, relieving pain, and preventing disability
• Allow patients and their families to maintain dignity and independencewhile receiving safe, effective rehabilitation in the security of their home
• Drive to patient homes in the assigned territories on daily basis.
