Qualifications

• 12 months Home Health OT experience in last 3 years required.
• HCHB charting preferred.
• Must have valid driver’s license and auto insurance.

Responsibilities

• Provide in-home Occupational Therapy services to patients in their homes.
• Evaluate patients and plan treatment to maximize independence.
• Develop and adjust treatment plans based on patient data.
• Deliver in-home OT programs to restore function.
• Drive to patient homes daily in Marshfield, WI and surrounding areas.

Pay Rate:Range $2200 to $2240 Weekly Gross pay based on a 40-hour work week ( Includes Non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers)

MileageReimbursement: $0.49/mile for distancecovered between patient home appointments

Unit: Home Health

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, No weekend or call

Position Requirements:

• Current Occupational Therapist experience: 1-2 years - Required
• Recent/Current Home Health OT experience: 12 months within the last 3 years - Required
• Home Care Home Base (HCHB)charting experience - Preferred
• Must have valid Driver’s Licenseand Personal Auto Insurance

Education, Licenseand Certification Requirements:

• Bachelor of Science degree in Occupational Therapy or Similar – Required

Job Description:

• Provide Occupational Therapy services to patients/clients at their homes.
  
• Utilize skills in patient/client evaluation and treatment, professionalconsultations to achieve maximum patient/client independence in accordance withthe individual's functional level and potential
  
• Plans and prepares treatment plan based on evaluation of patient data
  
• Provide in-home OT treatment programs to assist patients in restoringfunction
  
• Allow patients and their families to maintain dignity and independencewhile receiving safe, effective rehabilitation in the security of their home
  
• Drive to patient homes in the assigned territories on daily basis.
