The LVN performs patient care management services supporting the established plan of care as directed by licensed staff. This includes relaying instructions from physicians, collecting and documenting patient data, concerns, messages, and educational information within our operational platform. Providing leadership to support staff is also part of the role.

Qualifications
  • State issued license as a Licensed Vocational or Practical Nurse.
  • Graduate of an accredited nursing school: Licensed Vocational or Practical Nursing.
  • 2 years related experience or an advanced healthcare degree (e.g., bachelor's degree) required.
  • 3 to 5 years related experience preferred.

Physical Demand Level: Light.

This remote LPN position requires travel between our corporate location and other sites a few times a month. Candidates must reside in Colorado.
