Responsibilities
Job Summary

The Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) is responsible for demonstrating skills and behaviors that meet a fully competent level of performance during the orientation period. The LVN performs patient care management services supporting the established plan of care as directed by licensed staff within the team. This includes relaying instructions from physicians, collecting and documenting patient data, concerns, messages, and education within the care management platform. Providing leadership to support staff.

Essential Key Job Responsibilities
Assessment
  • Conduct screenings based on criteria, refer patients to healthcare programs, specialists, and multidisciplinary team members such as diabetes educators, home visit providers, geriatric clinics, home health, pharmacists, etc.
  • Escalate to RN, SW, or supervisor when patient needs or concerns are beyond scope of practice.
  • Assist patients and caregivers in achieving compliance and adherence to the care plan, notifying RN/SW or provider of issues, and collaborating with team members.
  • Coach patients, family, and caregivers about the disease process, recognizing signs of worsening conditions, and next steps. Identify cases to discontinue from the program and document rationale.
Leadership
  • Work independently with minimal supervision, demonstrating decision-making within policies and scope of practice.
  • Serve as advocate and liaison among patients, families, healthcare providers, and community resources.
  • Participate in departmental and organizational meetings, committees, and projects.
Critical Thinking
  • Monitor compliance with appointments, identify patterns of nonadherence, and coordinate scheduling.
  • Help patients navigate the healthcare system to ensure timely and appropriate care, escalating as needed.
Relationships
  • Develop effective relationships with healthcare team members, understanding diverse perspectives.
  • Proactively seek additional clinical input to ensure appropriate care/services.
Additional Responsibilities
  • Meet patients and families in community, home, or other locations, ensuring safety and escalating concerns.
  • Assist with research, audits, and process improvements as directed.
  • Support day-to-day operations, including hiring, mentoring, project participation, and data analysis.
Qualifications
  • Graduate of an accredited nursing school: LVN/LPN.
  • 2 years relevant experience or an advanced degree required.
  • 3 to 5 years experience preferred.
  • State-issued LPN license.
  • Experience managing patient populations, escalating issues, and working with community partners.
  • Strong organizational, communication, and computer skills, including proficiency in Microsoft Office.
