Responsibilities

• Participation in the development of program plans for individuals; monitor and revise as needed.
• Identify and coordinate services for individuals and families.
• Documentation, completing forms and reports per NLACRC policies.
• Advocate for individuals served by community agencies.
• Schedule and attend interdisciplinary meetings with individuals, families, providers, advocates.
• Identify barriers to service delivery and apply culturally sensitive strategies.
