NLACRC in California seeks a Service Coordinator to provide information, advocacy, and service arrangements for consumers and families. The role includes developing individualized program plans, coordinating services, and documenting all steps in NLACRC records.

You will advocate for clients, attend interdisciplinary meetings, and address barriers with empathy while respecting cultural needs. The position supports home, community, and office settings with a potential hybrid/remote option.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field.
• Master’s degree may substitute for experience.

Responsibilities

• Provide information, advocacy, and service arrangement for consumers and families.
• Develop and monitor program plans for individuals; revise as needed.
• Identify and coordinate services for individuals and families.
• Document, complete forms and reports per NLACRC policies.
• Advocate for individuals served by community agencies.
• Attend interdisciplinary meetings with individuals, families, providers, and advocates.
