Job Summary

Turenne PharMedCo is seeking a full-time Consultant Pharmacist for the Jackson, TN territory (West TN). The Consultant Pharmacist is a specialized role responsible for providing pharmaceutical expertise and consultation services to skilled nursing and other post-acute care facilities. This position requires a licensed pharmacist with a strong understanding of long-term care pharmacy regulations, medication management practices, and clinical pharmacy principles.

Key Responsibilities

1. Medication Regimen Reviews

Conduct routine reviews of residents' medication regimens to identify potential issues, such as inappropriate drug therapy, drug interactions, and duplicate therapy.
Collaborate with healthcare providers to develop and implement strategies to optimize medication therapy and improve resident outcomes.

2. Medication Reconciliation

Perform medication reconciliation processes to ensure accurate and up to-date medication lists for residents.
Verify medication orders upon admission, discharge, and transfer, and reconcile discrepancies as needed.

3. Regulatory Compliance

Ensure compliance with federal, state, and local regulations governing pharmacy services in long-term care facilities.
Provide guidance and support to facility staff on regulatory requirements related to medication management, documentation, and quality assurance.

4. Quality Assurance and Performance Improvement

Implement quality assurance programs to monitor medication use processes, medication-related outcomes, and adherence to standards of practice.
Participate in performance improvement initiatives to enhance medication safety, resident care, and overall quality of life.

5. Staff Education and Training

Provide education and training to facility staff on medication-related topics, including proper medication administration, storage, and documentation practices.
Offer guidance on medication safety protocols, error prevention strategies, and adverse drug reaction monitoring.

6. Consultation and Collaboration

Serve as a resource and consultant to facility administrators, physicians, nurses, and other healthcare professionals on pharmaceutical issues and best practices.
Participate in interdisciplinary team meetings and care conferences to contribute expertise on medication management and resident care.

7. Documentation and Reporting

Maintain accurate and up-to-date documentation of medication reviews, interventions, and consultations.
Generate reports and summaries of consultant pharmacist activities for facility administration and regulatory agencies as required.

Qualifications

Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program is required.
Current Tennessee pharmacy license in good standing is required. Additional pharmacist licensure with KY is preferred.
Experience in long-term care pharmacy practice, with a focus on consultant pharmacist services.
Strong knowledge of federal and state regulations governing pharmacy services in long-term care facilities.
Certification in Geriatric Pharmacy (CGP) or related specialty.
Experience with pharmacy consulting software and documentation systems.
Excellent communication, interpersonal, and problem-solving skills.

Education

Must possess and maintain any and all necessary active and unencumbered certifications, degrees, or licenses in the necessary state, to the extent the position requires such certifications, degrees, or licenses.

Working Conditions

Works in office area(s) and in a long-term care environment (i.e. nursing home, drug rooms, nurses’ stations resident rooms, etc.). Travel requirements are 100% with the possibility of 1-2 overnights per month.
Moves intermittently during working hours.
Is subject to frequent interruptions.
Is involved with customers, and customer representatives, family members, residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
Communicate with the designated facility staff, nursing personnel, residents, or family members.
Works beyond normal working hours and on weekends and holidays when necessary.
Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
Is involved in community/civic health matters/projects as appropriate.
Attends and participates in continuing educational programs.
Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS, COVID-19, and Hepatitis B viruses.
May be subject to the handling of and exposure to hazardous chemicals.

Must possess the ability to deal tactfully with customers, personnel, residents, family members, visitors, government agencies/personnel, and the general public.
