Job summary

Metropolitan Government of Nashville & Davidson County is seeking a Program Specialist 2 to perform administrative and professional duties within the Conservatorship and Burial Programs. The role involves obtaining conservatorship orders, requesting medical records, preparing case and medical summaries, and coordinating services for burial or cremation for eligible clients.

You will interact with Conservators, caregivers and respondents, conduct home assessments, and maintain accurate records

Qualifications

• Bachelor's Degree from an accredited college or university.
• Experience in de-escalation, interviewing and referrals.
• Experience in a social services related field.

Responsibilities

• Reviews and analyzes medical records within the Conservatorship Program.
• Compiles and submits medical summaries for Respondents.
• Keeps track of medical records and their status on-site.
• Coordinates with medical providers, Conservators, and care providers.
• Assists in Burial Program screenings and coordinates cremation/gravesite services.
