Responsibilities

• Provide case management and care coordination for clients in the facility.
• Coordinate continuum of care activities ensuring optimal resources and compliance.
• Conduct psychosocial assessments and individual, family and group sessions.
• Assist with treatment planning and discharge activities as needed.
• Develop, implement, manage and evaluate client care plans.
• Collect and relay clinical data to the treatment team.
• Communicate medical criteria between insurance providers and the treatment team.
