Job summary

Cheer Home Care is actively hiring Concierge Registered Nurses (RNs) to provide one-on-one, in-home care for older adults in La Jolla and North San Diego County, CA. This per diem role offers a pay range of $45–$80 per hour, with shifts centered on 12-hour day coverage, delivering premium, personalized care in clients' homes.

As part of an interdisciplinary team, you will assess, plan, implement, and manage individualized care plans while educating clients and families.

Qualifications

• Current, unrestricted California registered nurse license.
• 3 years of experience as a registered nurse.
• Home Health experience preferred.
• Proof of Negative TB result.
• Active California Driver's license and car insurance.

Responsibilities

• Comprehensive Assessments: conduct thorough clinical assessments of clients’ physical, emotional, and environmental needs.
• Care Planning & Goal Setting: develop customized, evidence-based care plans and goals for each client.
• Chronic Care Management: oversee ongoing management of chronic conditions with proactive interventions.
• Post-Surgical & Wound Care: provide wound care and medication management as needed.
• Client & Family Education: educate clients and families on diagnoses and treatment plans.

Key Responsibilities & Duties
