Cheer Home Care, LLC. is actively hiring a Concierge Registered Nurse to provide one-on-one in-home care in La Jolla and North San Diego County, CA. The role focuses on assessments, care planning, chronic care management, wound and post-surgical care, and family education.

We seek a California-licensed RN with at least 3 years of experience, preferably in home health. Qualifications include TB testing, recent physicals, a valid CA driver’s license, and reliable mobile access.

Qualifications

Current, unrestricted California registered nurse license.
3 years of experience as a registered nurse.
Home Health experience preferred.
Proof of Negative TB result.
Physical Screening Documentation within the last year.
Active California Drivers license and car insurance.
Cell phone with Internet Access.

Responsibilities

Comprehensive Assessments: Conduct thorough clinical assessments of clients' physical, emotional, and environmental needs.
Care Planning & Goal Setting: Develop customized, evidence-based care plans and establish measurable health and wellness goals for each client.
Chronic Care Management: Oversee the ongoing management of chronic conditions, ensuring proactive and preventative interventions.
Post-Surgical & Wound Care: Provide specialized skilled nursing interventions, including post-operative care, wound care, and medication management as needed.
