TheKey Concierge Nursing Services seeks an adaptable Home Health RN to serve clients across Los Angeles and Orange County, CA. The role involves in-person home visits, initial assessments, and quality assurance re-evaluations with shift-based expansion planned.

Per diem, on-call, and part-time opportunities offer flexible scheduling and autonomy. Qualified candidates have an active California RN license (BSN preferred), 2+ years of nursing experience (home health preferred), and a valid CA

Qualifications

• Active California RN license (BSN preferred).
• Minimum 2 years nursing experience, home health or elder care preferred.
• Flexible schedule; willing to work on-call, per diem, and accept last-minute assignments.
• Valid California Driver’s License, reliable vehicle and auto insurance.
• Strong clinical assessment, customer service, conflict resolution, and timely EHR documentation.

Responsabilités

• Conduct in-person initial assessments and reassessments to build tailored care plans.
• Perform skilled nursing visits and routine wellness checks per physician orders.
• Accept per diem visits on an as-needed basis, including last-minute requests.
• Engage clients and families as the main clinical contact with multidisciplinary teams.
• Complete timely clinical notes and ensure compliance with state home health regulations.
• Transition to block shift-work as case volumes grow in the region.
