Job summary

TheKey Concierge Nursing Services is seeking a Licensed Vocational Nurse (LVN) for a per diem/on-call/part-time role in Los Angeles, CA. You will conduct initial nursing assessments, develop and update tailored care plans, and coordinate services to ensure a seamless client experience.

Responsibilities include delivering direct nursing care, educating clients and families, and maintaining compliance with state home care regulations.

Qualifications

Licensed Vocational Nurse (LVN) in the state of employment (active).
Minimum 2 years in healthcare, eldercare, home care, or related clinical field preferred.
Proficient in completing and submitting accurate clinical documentation in an electronic health record system.
Strong customer service, communication, and conflict-resolution abilities.
Valid driver's license and proof of current auto insurance.

Responsibilities

Client Onboarding & Assessment: Initiate prompt client outreach and schedule initial meetings with clients and families.
Conduct nursing assessments: perform in-person assessments, reassessments and home safety evaluations.
Develop tailored care plans translating assessments into individualized nursing plans.
Care Coordination & Clinical Management: Deliver direct nursing care per the established plan of treatment.
Coordinate services to ensure a seamless client experience.
Monitor changes in condition with regular home visits and timely reassessments.
Educate clients and families with ongoing health instruction and support.
Client Retention & Experience: Build rapport and maintain an exceptional client experience.
Support long-term retention through proactive communication and appreciation initiatives.
Provide onboarding support during new client case launches.
Compliance, Documentation & Operations: Ensure regulatory compliance and maintain documentation per policy.
Prepare and submit timely clinical notes and documentation.
Respond to inquiries within established timeframes.
Monitor and review client service rates as needs evolve.
General & After-Hours Support: On-call availability and after-hours support as required.
Perform other duties to support clinical and business goals.
