Job summary

Under supervision of Care Management Manager, you will advocate for patients, ensure continuity of care, and optimize resource use in a health-care setting. Preferred qualifications include ACM/CCM certification and Washington Clinical Independent Social Worker Associate license.

Qualifications

• Master's Degree in Social Work or related field.
• Upon hire: Washington Clinical Independent Social Worker Associate License.
• 3 years of Social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/ care coordination.

Responsibilities

• Coordinate care for complex designated patients from pre-admission to immediate post-discharge.
• Promote coordination of care, continuity of care, and achievement of psychosocial outcomes.
• Advocate for patients and manage resources efficiently.
