Job summary

Qualifications

• 5+ years of care management experience in acute or ambulatory settings.
• BSN for nurses or LMSW/LCSW licensure for social workers.
• Strong knowledge of CMS guidelines and utilization management.

Responsibilities

• Conducts EMR reviews and patient interviews to identify care gaps.
• Performs readmission reviews and helps reduce avoidable hospitalizations.
• Facilitates complex discharge planning and transitions of care.
• Analyzes utilization metrics to identify trends and opportunities for improvement.
• Collaborates with physicians, payers and the care team to coordinate services.
