Sinai Hospital, part of LifeBridge Health, is seeking a Complex Discharge Planning Coordinator to coordinate complex discharge planning, manage high-need cases, and ensure safe transitions of care for patients in Baltimore. You will oversee caseloads up to 15 patients, collaborate with an interdisciplinary team, and identify opportunities for early intervention to reduce delays and costs.

A sign-on bonus of up to $10,000 is offered.

Qualificações

• Master's degree in Social Work required.
• LMSW or LCSW-C licensed.
• 3 years inpatient discharge planning experience or 2-3 years in complex case management.

Responsabilidades

• Coordinate complex discharge planning for difficult-to-place, high-cost, and complex patients.
• Manage caseloads of up to 15 patients with escalation process and collaboration with interdisciplinary team for safe discharge planning.
• Monitor patients to identify opportunities for early intervention and reduce discharge delays and costs.
