Position Overview

Licensed Social Worker (LSW) sought to work as a part of an interdisciplinary complex care management team. The LSW will report to the RN Complex Care Manager and will be an integral part of the Specialized Health Services interdisciplinary team across the PHMC nurse-managed Health Network sites. The LSW will work at these health centers and in the community with diverse and underserved patient populations. The LSW will provide community-based clinical social work, case management, and outreach services to help coordinate care and improve patient health outcomes. This position provides clinical hours towards the LCSW. The LSW will receive clinical supervision from an LCSW within the Health Network, along with training and support for the social services team.

Responsibilities
  1. Provide multidisciplinary assessments evaluating all relevant information to determine needs and barriers in healthcare, social supports, and behavioral health services.
  2. Participate actively as a member of the interdisciplinary health care team (IDT) by providing input and developing strategies for health, behavioral health, and psychosocial issues.
  3. Collaborate with medical and auxiliary providers to develop healthcare plans based on identified needs.
  4. Engage with clients through outreach, eliminating barriers related to social, cultural, linguistic, and educational factors to promote self-care.
  5. Facilitate communication and build positive relationships between clients and medical providers.
  6. Ensure clients access appropriate health and social services in a timely, coordinated manner, advocating on their behalf.
  7. Provide individual counseling including functional and strength-based assessments, psychoeducation, medication adherence, disease self-management, motivational interviewing, problem-solving, and substance use evaluation.
  8. Document all services promptly in the electronic health record system.
Skills
  • Thorough knowledge of community resources.
  • Understanding of DSM-5 behavioral health conditions and chronic diseases.
  • Strong clinical judgment, analysis, and critical thinking skills.
  • Excellent time management and attention to detail.
  • Interpersonal, organizational, and communication skills.
Experience
  • Medical literacy.
  • Experience in behavioral medicine and health psychology.
  • Teamwork in interdisciplinary settings.
  • Experience with evidence-based interventions like CBT, motivational interviewing, SBIRT.
  • Crisis intervention and substance abuse experience preferred.
  • Experience working with diverse populations.
Education Requirements
  • Master's Degree in Social Work from an accredited institution.
  • Licensed Social Worker (Pennsylvania).
Physical Demands
  • Standing/walking for 2/3 of the time, sitting for 1/3, use of hands for 2/3, reaching under 1/3, stooping/kneeling/crouching under 1/3, talking/hearing over 2/3, lifting up to 10 lbs. occasionally.
Work Environment
  • Moderate noise typical of office settings. Exposure to bloodborne pathogens requiring PPE.
Salary

Commensurate with education and experience.
