Job summary

The Lake County Health Department and Community Health Center is seeking a Community Health Worker to support the Complex Care Management program within the Physical Health team. This role partners with an RN care manager and a complex care coordinator to identify, engage and support complex patients, improving outcomes and reducing unnecessary ED visits.

Qualifications

• High School Diploma required.
• Valid driver’s license and ability to travel within the county.
• Experience in community work preferred (advocacy, volunteering, or organizing).
• Spanish or other language fluency preferred.

Responsibilities

• Complete all required training sessions and activities.
• Use outreach tools and techniques to contact patients and engage them in the Complex Care Management program.
• Build rapport and connection with patients in an empathetic manner.
• Collaborate with patient (and family/advocate at patient request) to understand their health care needs and goals; develop a care plan.
• Review care plan with patient at every contact for progress and escalation needs.
• Maintain records of care management activities and participate in care reviews with the PCPs as needed.
• Screen patients for health and Social Determinants of Health issues and make service referrals.
