Job Opportunity: Nurse Practitioner / Physician Assistant in Middlesex County, CT

Compensation: $40,000 Student Loan Repayment or $20,000 Sign-on Bonus for eligible individuals.

This role involves providing home-based medical care within an interdisciplinary team, working closely with patients, their families, and other healthcare professionals to develop and implement personalized care plans, and participating in quality improvement initiatives.

Primary Responsibilities:
  • Perform comprehensive assessments and ongoing care for enrolled patients
  • Develop and update patient management plans
  • Educate patients and their families about health conditions and wellness strategies
  • Coordinate with healthcare teams following hospital or emergency transfers
  • Participate in quality initiatives and maintain current knowledge through continuing education
  • Practice in accordance with state laws and regulations
Qualifications:

Required:

  • Completion of an accredited Nurse Practitioner or Physician Assistant program
  • Current state RN/NP or PA license
  • Certification from AANP, ANCC, or NCCPA
  • Reliable transportation and compliance with safety policies

Preferred:

  • 2+ years of relevant clinical experience
  • Knowledge of care systems, coding, and standards
Additional Details:

The salary range for Connecticut residents is $88,000 to $173,200 annually, based on various factors. Benefits include comprehensive packages, incentive programs, stock purchase options, and 401k contributions. Employees must remain in the role for at least 12 months, with a 36-month commitment to fully benefit from student loan repayment programs.
