Positions in this role function as provider members within the HBMC interdisciplinary team, delivering home-based medical care within an assigned geography. The role focuses on improving patient health and well-being by collaborating with primary care providers, interdisciplinary teammates, consultants, and, when necessary, the health plan Medical Director and other health professionals to develop and implement patient care goals throughout their journey.


Primary Responsibilities:


  1. Perform initial comprehensive assessments for all newly enrolled patients and provide ongoing care.
  2. Develop and update care plans as needed, especially following hospitalizations or condition changes.
  3. Meet with patients and/or their legal representatives to review care plans; involve the PCP or supervising physician when applicable.
  4. Perform basic nursing procedures, including IV placement, blood draws, injections, Foley catheter management, wound care, etc. Training provided.
  5. Prescribe diagnostics and interventions to prevent unnecessary hospital admissions.
  6. Conduct follow-up and post-discharge assessments per SOP.
  7. Consult with hospital, emergency, or post-acute clinical teams following patient transfer notifications.
  8. Educate patients and their representatives about disease management and wellness promotion.
  9. Participate in meetings related to patient care and clinical excellence.
  10. Implement quality initiatives like HEDIS measure campaigns to improve care standards.
  11. Be on-call during business hours and participate in after-hours on-call rotations.
  12. Stay updated on medical and nursing research, technology, and related issues through continuing education.
  13. Practice according to state laws and regulations governing advanced practice nursing or physician assistant roles.

Required Qualifications:


  1. Completion of an accredited Nurse Practitioner or Physician Assistant program.
  2. Current state RN/NP or PA license.
  3. Certification from AANP, ANCC, or NCCPA.
  4. Reliable transportation and compliance with Optum Motor Vehicle Safety policy if driving.

Preferred Qualifications:


  1. 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative, or emergency/urgent care.
  2. Knowledge of long-term care, community resources, and cost-effective care options.
  3. Familiarity with CPT, ICD-10, HCPCS codes.
  4. Knowledge of clinical standards and regulations like NCQA, CMS, Medicaid/Medicare.

Connecticut Residents Only: Salary ranges from $88,000 to $173,200 annually, based on various factors. Benefits include comprehensive packages, incentives, stock purchase plans, and 401(k). Details subject to eligibility.


Note: Employees must be active in their role for at least 12 months; student loan benefits require a 36-month commitment within the role.
