Qualifications

• Bachelor’s or Master’s degree in Social Work from an accredited college or university.
• Knowledge of the medical case management process and the needs of geriatric patients.
• Working knowledge of healthcare funding resources such as Medicare Parts A, B, C, and D, Medicaid, Managed Care, and private insurance.

Responsibilities

• Work with individual patients and families as they adjust to center life and cope with illness, disability, and end-of-life concerns.
• Serve as an advocate for patients and families in expressing, defining, and resolving concerns or grievances
• Coordinate discharge planning functions and assist with safe transitions of care
• Participate in developing patient care plans in cooperation with other healthcare disciplines
• Communicate the current status and needs of Social Services to the Director of Social Services
• Assist patients and families with understanding available healthcare and funding resources
• Maintain confidentiality of patient and professional information
• Work cooperatively with nursing, therapy, administration, and other members of the interdisciplinary team
