Job summary

Windsor Gardens Care Center is seeking a Social Worker to plan and administer social service programs for residents and families. The role will coordinate discharge planning, assess social needs, and liaise with the care team and external resources to ensure comprehensive support.

The position requires a Bachelor’s in Social Work or related field and knowledge of New Jersey regulations. Strong communication and collaboration with families and staff are essential.

Qualifications

• Bachelor’s degree in Social Work or related field (Sociology, Psychology, Rehabilitation Counseling).
• Knowledge of New Jersey State social service regulations and required certifications.
• Experience with family assessment and interventions preferred.

Responsibilities

• Plan and administer social services programs for residents and families.
• Identify residents’ medical-related social needs and provide appropriate services.
• Coordinate discharge planning and community resource referrals.
• Maintain records of social work consultations and services provided.
• Conduct family meetings to discuss care planning and discharge options.
• liaison with residents, families, outside agencies, and staff to protect residents’ rights.
