Complete Care at Sheridan Commons in Chicago, IL is hiring a Part-Time Social Services Assistant for the day shift. You will join an interdisciplinary team to support residents’ psychosocial, emotional, and social needs and ensure holistic, high-quality care during their stay.

The role requires an Illinois LSW/CSW license, experience in long-term care, and proficiency with EMR systems. You will advocate for residents' rights and assist with transition planning and community resources.

Qualifications

LSW / CSW social work license in Illinois.
Experience in long-term care, discharge planning, resident advocacy, and knowledge of regulations.

Responsibilities

Identify residents' psychosocial, mental, and emotional needs.
Complete social histories and psychosocial assessments for residents.
Collaborate with interdisciplinary team to advocate for residents' rights.
Maintain a safe environment; address concerns per facility policies and reporting guidelines.
Assist residents and families with transition planning and community resources.
Document accurately within the EMR system.
Perform other related administrative duties as assigned.
