Job summary

Napa Valley Care Center is seeking a compassionate Speech-Language Pathologist to join our rehabilitation team in Napa, CA. Qualifications

• Master's or Doctoral Degree in Speech-Language Pathology.
• Active California Speech-Language Pathologist License in good standing.
• Strong clinical, documentation, and communication skills.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Evaluate residents referred for speech, language, cognitive, and swallowing disorders within established timeframes.
• Develop individualized treatment plans and obtain physician approval as required.
• Provide skilled speech-language pathology services per physician orders and standards.
• Assess and treat communication, cognition, voice, language, speech, and dysphagia disorders.
• Collaborate with physicians, nursing, rehab staff, residents and families to optimize outcomes.
• Participate in care plan meetings, patient conferences, and team discussions.
• Educate residents, caregivers, families and staff on treatment goals and techniques.
• Complete timely, accurate documentation in EMR systems.
• Assist with discharge planning and caregiver training.
• Supervise SLPAs and CFYs where applicable.
• Support ongoing staff education through in-service training.
