Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Speech Language Pathologist (SLP) to evaluate and treat residents with speech, language, voice, and swallowing disorders within our rehabilitation team.

The SLP will develop individualized treatment plans, provide therapy services, monitor progress, and collaborate with physicians and nursing staff to support resident independence and quality of life.

Qualifications

• Active Florida SLP license in good standing.
• CCC preferred but not required.
• Experience in skilled nursing or rehabilitation settings preferred.

Responsibilities

• Evaluate residents with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized treatment plans based on resident needs and goals.
• Provide speech therapy services to improve communication, cognition, and swallowing function.
• Monitor resident progress and adjust treatment plans as appropriate.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to support positive resident outcomes.
• Maintain accurate and timely clinical documentation per regulatory and facility standards.
• Educate residents and families regarding therapy programs and goals.
• Participate in care planning and discharge planning processes.
