The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Speech Language Pathologist (SLP) to join our rehabilitation team. The SLP will evaluate and treat residents with speech, language, communication, voice and swallowing disorders, helping them achieve independence and quality of life.

Qualifications

• Active Florida SLP license in good standing.
• CCC certification preferred.
• Experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary team.

Responsabilités

• Evaluate residents with speech, language, cognitive, communication and swallowing disorders.
• Develop and implement individualized treatment plans based on resident needs and goals.
• Provide speech therapy services to improve communication, cognition and swallowing function.
• Monitor resident progress and adjust treatment plans as appropriate.
• Collaborate with physicians, nursing staff and interdisciplinary team members to support resident outcomes.
• Maintain accurate and timely clinical documentation in accordance with standards.
• Educate residents and families regarding therapy programs and goals.
• Participate in care planning and discharge planning processes.
• Ensure compliance with federal, state and facility policies and procedures.
• Maintain a safe, organized therapy environment.
