Job summary

Oakmont Management Group is seeking a Care Provider at Ivy Park in Santa Rosa, CA to assist residents with daily living activities and support independence. The role emphasizes compassionate care, safety, and communication with the care team and families.

Responsibilities include assisting with ADLs, responding to resident needs, reporting changes to supervisors, maintaining a safe environment, and collaborating with colleagues to ensure high-quality care.

Qualifications

• Experience preferred but not necessary; we will train the right candidate.
• Must be at least 18 years of age.
• High School Diploma or GED.
• Strong desire to work with the elderly and care for their needs.
• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests.
• May require vaccination and use of PPE to prevent spread of diseases per regulatory guidelines.

Responsibilities

• Assist residents in Activities of Daily Living (ADL) as per the care plan.
• Respond promptly to residents' needs while maintaining dignity and safety.
• Communicate observed changes of condition to a supervisor.
• Maintain a safe and secure environment following safety standards.
• Encourage teamwork through cooperative interactions with staff and departments.
