Responsibilities

• Performs routine physical/psychosocial assessment to collect basic data and re-evaluate patients.

• Adapts teaching strategies to individual patients and families within the care plan.
• Ensures efficiency in care delivery, determining priorities and providing solutions.
• Identifies patients for ambulatory care services and special needs; supports access to care.
• Advocates for patient rights and seeks resolution of conflicts.
